Slack Forms: Get Every Submission Where Your Team Already Is

Slack forms are only useful if someone sees the answers. Build the form in Ovoform, and every completed submission posts a message to the Slack channel you choose, with the answers that matter and a button that opens the full response.

No credit card required. Unlimited responses. The Slack integration is included with Business and Enterprise plans.

What you can do with Ovoform and Slack

Alert

A channel message for every submission

As soon as a response is saved, Ovoform queues a message to the Slack channel you picked. Public and private channels both work, and Ovoform can join a public channel by itself if needed.

Choose

Which answers appear

By default the message shows the first five answers. Pick the questions you want instead, so a sales alert leads with the company and budget rather than the whole form.

Route

Different channels for different answers

Add a Slack action to a form workflow and give it conditions. A low CSAT score can go to the support channel while a large deal goes to sales, all from the same form.

Open

The full response in one click

Every message carries a View Full Response button, so a teammate can jump from the alert to the complete submission.

How the Slack connection works

  1. Connect your Slack workspace

Install the Ovoform app in Slack. A Slack workspace admin may need to approve the installation.

  2. Choose a channel

Pick the channel that should receive submissions. For a private channel, invite the Ovoform app to it first.

  3. Select the answers to show

Choose which questions appear in the message, or keep the default of the first few answers.

  4. Publish and test

Submit a test response and confirm the message arrives in the channel.

What syncs, and what to know

Ovoform connects to Slack as an app with a bot token, and asks only for the permissions it needs to post messages and to list the channels you can choose from. It posts a formatted message with a header, the form title, the submission date, the selected answers and a View Full Response button. Each completed response is queued after it is saved, so respondents never wait on Slack. If Slack rejects a post, Ovoform retries twice, roughly one and three minutes later. Long answers are truncated so a single response cannot overflow the message, and payment answers show their status only.

Routing by answer works through workflows. In a form's Workflow tab, add a Slack action, pick the workspace and channel, and set the conditions under which it runs. Conditions can test answers, hidden fields and URL parameters. The action can also carry your own message text with answer values filled in. The basic Slack connection sends everything to one channel, so use workflow actions when different answers need different destinations.

Some things are not supported. Messages go to channels, not to individual people, so there are no direct messages. Ovoform does not post in threads and does not mention people or the whole channel. You can connect more than one Slack workspace and choose which one each workflow action posts to. A resync posts earlier responses one at a time, so use it with care in a busy channel.

If something goes wrong, the integration shows an error status in Ovoform, and the respondent still sees your normal thank-you screen. For a private channel, invite the Ovoform app before you connect, because it can only post where it is a member. If Slack revokes the app's access, syncing stops until you reconnect, and responses that were not posted can be sent again with a resync.

Frequently Asked Questions

Yes, through workflows. Add a Slack action in the form's Workflow tab, choose a channel and set conditions on answers, hidden fields or URL parameters. You can add several actions with different conditions. The basic Slack connection, without a workflow, sends every response to a single channel.

By default, the first five answers. You can choose exactly which questions to include instead. The message also shows the form title and the submission date, and it ends with a View Full Response button. Very long answers are truncated, and payment answers show only their status.

Channel posts only. Ovoform does not send direct messages to individual people. Public and private channels both work, but for a private channel you need to invite the Ovoform app first. To reach one person, create a small private channel that only they are in.

No. Each submission is posted as its own top-level message in the channel, and Ovoform does not use threads or mentions. If a busy channel gets noisy, send the alerts to a dedicated channel that people can mute or follow as they prefer.

Yes. You can connect several Slack workspaces to your Ovoform account, and each workflow action chooses which workspace and channel it posts to. One form can therefore notify channels in more than one workspace, using more than one action.
